Many people assume that every immigration applicant must complete an Immigration Medical Exam (IME) in Canada. In practice, not everyone is required to. Whether you need one depends on several factors: the type of permit or visa you are applying for, how long you plan to stay in Canada, your country of citizenship or travel history, and your intended occupation.

The Key Triggers for Requiring an IME

IRCC requires an immigration medical exam when at least one of the following applies:

• You intend to stay in Canada for more than six months.
• Your occupation involves working with vulnerable populations (healthcare, childcare, in-home care for the elderly or disabled).
• You have lived in or travelled to a designated country or territory for six months or more consecutively in the year before coming to Canada.
• Your application type or stream specifically requires an IME.

Who May Be Exempt from the IME?

Applicants who do not meet any of the triggers above may not need a medical exam. Common scenarios where no IME is required include:

• Temporary residents staying less than six months who are not working in a designated occupation and are not from a designated country.
• Short-term business visitors who meet the above criteria.
• Some applicants renewing permits or extending status from within Canada, depending on their circumstances.

How to Confirm Whether You Need an IME

The most reliable ways to confirm:

1. Review your IRCC instruction letter — if IRCC requires an IME, they will say so explicitly.
2. Use IRCC's online tool for checking medical requirements based on your application type.
3. Consult an authorized immigration representative if you are unsure.

Do not assume you are exempt. If your situation meets any of the triggers, you will need an IME.

What Happens If You Skip a Required IME?

If you need an IME but do not complete one, your application may be placed on hold or refused. IRCC will not process applications missing required medical exam results.
