What Is an Immigration Medical Exam?

An Immigration Medical Exam (IME) is a mandatory health assessment required by Immigration, Refugees and Citizenship Canada (IRCC) for most permanent residence, refugee, and certain temporary residence applications. The exam must be performed by an IRCC-accredited Panel Physician. Your own family doctor cannot conduct it.

At IMMEDEX, every exam is performed by a certified IRCC Panel Physician and includes everything IRCC requires: a physical examination, blood work, and chest X-ray, all completed in a single visit at our Scarborough clinic.

Chest X-Ray: Separate Payment Required

The $50 chest X-ray fee is paid directly to VPM Imaging at the time of your visit. Payment for the X-ray is accepted by cash or e-transfer. This fee is waived for eligible refugees.

When Is Additional Testing Required?

Recall: If your Panel Physician notices an abnormality during your exam, such as an irregular result in your blood work or a concern on your chest X-ray, they will contact you to schedule a Recall appointment. The Recall fee is $40.

Furtherance: If IRCC reviews your submitted results and requires further investigation, they will instruct you to contact your original Panel Physician to schedule a Furtherance appointment. The Furtherance fee is $90. Note that IRCC initiates this request, not the clinic.

Neither a Recall nor a Furtherance is common for healthy applicants, but it is worth factoring both into your budget as a contingency.

Does OHIP Cover an Immigration Medical Exam?

No. The immigration medical exam is not covered by OHIP or any provincial health insurance plan. It is a government-mandated immigration process, not a standard medical service, and the full cost is borne by the applicant.

There is one exception: eligible refugees with valid government-issued ID receive their complete immigration medical exam, including the chest X-ray, at no cost. This is covered through a federal government program.

*Chest X-ray is typically performed on-site concurrently with your medical examination at VPM Imaging, at an additional cost of $50 (waived for eligible refugees).

Why Do Some Applicants Pay More?

A few factors can affect your total cost:

01

Age

Applicants aged 75 and over pay $225 (vs. $200 for those aged 15–74), reflecting the more detailed assessment appropriate for seniors. Children aged 0–14 pay $160, and their exam does not require a chest X-ray until age 11.

Ages 0–14 → $160 Ages 15–74 → $200 Ages 75+ → $225
02

X-ray requirement

Applicants aged 0–10 do not require a chest X-ray. Those aged 11 and over do, adding $50 to the total.

Ages 0–10 → not required Ages 11+ → +$50
03

Follow-up appointments

If a Recall or Furtherance appointment is requested, additional fees apply. Most straightforward applications involve only the initial exam.

Recall → +$40 Furtherance → +$90
04

Multiple applicants

If you are sponsoring a spouse, bringing dependent children, or applying for parent sponsorship, each family member must have their own exam and pay their own fee.

Who Needs an Immigration Medical Exam?

You will need an IME if you are applying for:

Permanent Residence

Express Entry, Provincial Nominee Programs, and all other PR streams.

Family Sponsorship

Spouse, common-law partner, dependent children, parents, and grandparents.

Super Visa

For parents and grandparents visiting Canada for extended stays.

Work & Study Permits

If you've lived in certain countries for 6+ consecutive months.

Refugee Status

Required as part of the refugee protection application process.

New as of August 2025: Express Entry applicants must complete an upfront medical exam before submitting their permanent residence application, rather than waiting for IRCC instructions.

How Long Are IME Results Valid?

Your immigration medical exam results are valid for 12 months from the date of the exam. If your application is not finalized within that period, you may be required to complete a new exam. Plan your timing accordingly, particularly if your application processing is expected to take longer than usual.
