When preparing for your Canadian immigration medical exam, you will likely have questions about the blood tests involved. Understanding what is being tested, why, and what the results mean for your application can help you approach the exam with confidence.

Which Blood Tests Are Required?

IRCC requires two specific blood tests as part of the Immigration Medical Exam (IME):

• HIV screening: Tests for the presence of HIV antibodies. Required for applicants 15 years of age and older.
• Syphilis serology (RPR): Tests for syphilis. Required for applicants 15 years of age and older.

These are the only blood tests IRCC mandates as part of the standard IME. If your panel physician identifies a health concern warranting additional testing, they may request supplementary blood work — but this is case-by-case, not a standard requirement.

Why HIV and Syphilis?

Both conditions fall under IRCC's public health screening mandate. The goal is not to exclude applicants with these conditions by default, but to screen for conditions relevant to admissibility or public health planning. The admissibility determination based on any positive finding is made by IRCC, not the panel physician.

Does a Positive Result Mean Refusal?

Not automatically. HIV status alone is not grounds for medical inadmissibility in Canada. However, IRCC may assess whether the anticipated cost of treatment would place excessive demand on Canada's health services. A positive result triggers a review process, not an automatic refusal. For syphilis, a positive RPR typically leads to confirmatory testing; treated syphilis with negative confirmatory tests is generally not an admissibility concern.

What Happens with Your Blood Test Results?

Blood samples are processed at the laboratory used by your panel physician clinic. Results are included in your medical file submitted to IRCC through eMedical. You will not typically receive a separate lab report.

What Is the Urine Test For?

While not a blood test, the urine analysis is part of the standard IME. It screens for protein (possible kidney disease), glucose (possible diabetes), and blood in urine (kidney or bladder conditions).

Age Thresholds

• Under 11 years: urine test only (no blood test, no chest X-ray).
• 11 to 14 years: urine test + chest X-ray (no blood test).
• 15 years and older: urine test + chest X-ray + blood tests (HIV and syphilis).
